Online Pilates Disclaimer Form

Please complete this form before attending your first class.

Classes begin on May 30th and will run on Saturday mornings from 10 – 10.30am and Tuesday and Thursday afternoons from 4.30 – 5pm.

Disclaimer Statement

Although Pilates is very safe, I understand that any exercise involves a risk of injury. I agree to take responsibility for my participation in Pilates classes, and agree that I exercise at my own risk. I agree to follow my teacher’s instructions regarding safety & technique, & I understand that I should work at my own pace, respect my body, & stop to rest if I need to. I agree to inform my teacher immediately if I experience any pain or discomfort whilst exercising.

Pain is the body’s warning system & should not be ignored! I also agree to let my teacher know if I am pregnant or have any health issues/conditions that may impede safe exercise. I hereby declare that I will hold neither Bodywise Pilates ltd (or the teacher provided by them) nor Wessex Care responsible for any injury I may experience during these classes.
